Making claims easier

Making a claim is quick and easy. Simply submit your claim here by filling out the form. You also need to attach a medical certificate.

Our partner at Agile Underwriting will process the claim and a confirmation email and claim number will be sent to your nominated email address.

If additional information is required you will receive an email from Proclaim.

What information do I need to make a claim?

It is important that you obtain as much documentation as possible at the time the situation occurs, as it can be difficult to obtain some documents.

Get a written medical report or certificate from your treating doctor or specialist that clearly explains the medical condition.

Keep originals of all documents that you submit electronically.

Lodge your claim as soon as is reasonably practicable of the situation that gives rise to your claim.

How long does it take to process a claim?
